Patient Registration and Info
Please bring a list of your prescribed medications with you on your first visit to our office. Your doctor will want to review all medications. When you need to refill your ophthalmic prescriptions, please notify your pharmacist as early as possible. He or she will contact our office for the appropriate authorization.
Insurance & Fees
We participate with most health care plans. Please bring your current insurance card with you at the time of your visit.
We accept assignment from Medicare and many commercial insurances, HMO’s and PPO’s. Please contact your insurance company to verify our participation with your insurance plan if you have any concerns.
Please be sure to have the proper referral from your primary care physician for each office visit if required by your plan. Most insurance companies cover the medical portion of an eye exam if a medical complaint is verified.
Payment is expected at the time of service for co-payments or non-covered services. We try to make your visit as convenient as possible by submitting your claims for each visit, and making your visit hassle-free. Payments may be made by cash, checks, Visa, MasterCard, and the Discover Card.
